Contract for Services
This form must be completed and on file at our offices before requesting services.

Request for Notice to Owner
Use this form when you need to request a Notice to Owner. If you prefer you can go to our  Request for Notice to Owner Page and request your Notice to Owner online 

Claim of Lien Authorization
This form is required for ordering Claim of Lien preparation online.  Download the form, sign it and send the original to our offices.  You will then be able to request your Claims of Lien online

Request for the Recording of a Claim of Lien
Use this form when you need to File a Lien on a project.  If you prefer you can go to our Request for Claim of Lien Page and request your Claim of Lien online.  The Claim of Lien Authorization above must be on file at our offices prior to using this method.

Request for the Notice of Performance and Non-Payment (PUBLIC PROJECT)
Use this form if you are on a public bonded job and have not been paid.

Request for Notice of Non-Payment (PRIVATE PROJECT)
Use this form if you are on a private bonded job and have not been paid.

Notice of Commencement
The Notice of Commencement must be recorded with the Clerk of the Courts in the County where the job is being done not later than 90 days from the beginning of the job.

Claim of Lien
The Claim of Lien must be recorded not later than 90 days from your last day on the job.  Punch List and Warranty work do not extend the 90 day period.

Notice of Non-Payment
The Notice of Non-Payment is used when you are on a private, bonded job.  It must be received by the Prime Contractor and by the Surety (Bonding Company) not later than 90 days from your last day on the job.

Notice of Performance and Non-Payment
The Notice of Performance and Non-Payment is used when you are on a public, bonded job. It must be received by the Prime Contractor and by the Surety (Bonding Company) not later than 90 days from your last day on the job.

Conditional Partial Release of Lien
The Conditional Partial Release of Lien “conditions” your release by containing language that offsets the effect of the Release if, for whatever reason, you do not actually received payment, ie: the check you are given is insufficient, or a promise of payment is not met.  Florida Notice Corporation STRONGLY suggests that ALL Releases of Lien you offer, partial or final, contain this conditional language.

Statutory Release of Lien
This is the statutory release of lien as provided by Florida’s legislature.

Statutory Partial Release of Lien
This is the statutory partial release of lien as provided by Florida’s legislature.

Release and Satisfaction of Recorded Claim of Lien
This is the form used after a Claim of Lien has been properly recorded and served.

Lienor’s Sworn Statement of Account
The Sworn Statement of Account must be provided within 30 days when properly requested.

Lienor’s Demand on Owner
The Lienor’s Demand on Owner is used to obtain copies of contracts of those dealing directly with the Owner of the job.
